STScI Problem Report System Registration

Welcome to the STScI Problem Report System. This form is used to request an account so you can connect as an authenticated user and have access to more of the problem report functions. To register for an account, you must complete this form. The information will be saved and the account will be created at a later time. You will receive notification by email or by phone when the account is created. Users outside the institute should have a local sponsor so that we can verify the purpose for the account.

Data contained herein is ITAR controlled, and as such, release/disclosure of this data to a foreign person as described in 22CFR 120.16 is unlawful without the proper authorization of an exemption, export license or export agreement defined in 22CFR 120-130. Account access to this system is limited to US persons with an email address in the stsci.edu domain, the hst.nasa.gov domain, or persons with an NGIN account issued by the JWST project.
